Collection Of Names & Contact Information

 

Under Schedule 4 of the Reopening Ontario Act (even while in the Roadmap Exit Step) I am required to provide Durham Region Health Department (DRHD) with the names and contact information (phone numbers) of attendees (participants & spectators) of the sporting event. These records will be disclosed to the medical officers of health or inspectors under the Health Proctection Promotion Act for contact tracing associated with a positive COVID-19 case, within 24 hours of the request, or other time specified.
